Step-by-step instructions to view and close care gaps directly in your EHR
When users are viewing a patient chart or schedule within your EHR, they may see an [EHR] icon on the right side of their EHR screen. This icon indicates that Vim has surfaced additional patient insights, powered by the patient’s health plan(s).
Clicking the [EHR] icon opens Vim’s panel, where you’ll see a list of applications that provide patient insights and data, like quality measures and risk adjustment coding opportunities pulled from the patient’s health plan(s).
Each gap includes:
End users can click on the necessary, prompted actions to close a gap in today's visit (e.g., order a test, schedule a screening). The user then documents that the gap is already closed in the patient's chart.
The user doesn’t need to submit anything separately. Their actions in the EHR will be automatically shared with the health plan if the gap is closed.
Once reviewed, simply close the panel and return to the standard workflow. This is designed to be lightweight and embedded with no extra portal login, no switching tabs.
