What is a gap in Care Insights?

Vim’s Care Insights solution displays gaps and actionable patient information, like quality measures and risk adjustment coding opportunities at the point of care based on clinical guidelines and health plan data that are unique to the patient in front of the provider.

Where does the gap data come from?

The gaps that are shown in Care Insights are provided directly by participating health plans and provider networks, like ACOs or MSOs. Vim connects to these data sources through secure integrations to surface actionable gap information at the point of care.

Is the data secure and HIPAA-compliant?

Why might I not see gaps for all my patients?

Not all patients are covered by participating health plans and provider networks. You’ll only see gaps for patients whose data sources connected through Vim.


How accurate is the gap data?

Vim shows the most recent care gap data received from payers. That said, gaps are only as accurate as the payer's records. If you believe a gap is incorrect, document it in your EHR using your normal workflow and flag this discrepancy to the Vim Network Team.


Are there financial incentives tied to closing gaps?

Sometimes. Many payers offer incentives for quality gap closure (e.g., HEDIS, Stars, Risk Adjustment, ACO metrics). If your practice participates in a value-based care program, closing gaps in Vim may contribute to those quality scores.

Contact your network or payer rep to confirm which incentive programs you’re part of.


How often is the data refreshed?

Gap data is refreshed on a regular basis by each participating payer—often monthly or quarterly, depending on the plan.
